Apply for Medicaid or MBIWD |
- Online (you need to specify if you are applying for MBIWD), or
- By filling out the combined application form (other languages) and submitting it to your County Department of Job and Family Services (CDJFS) office (for MBIWD, fill out the Addendum form as well).

Note: If you get Supplemental Security Income (SSI) benefits, you automatically get Medicaid coverage and do not need to apply separately.
