• You can apply for SSI, SSDI, CDB, and Child’s Benefits at your local Social Security office or by calling 1-800-772-1213 or 1-800-325-0778 (TTY).
• To apply for Medicaid, Food Assistance, or Ohio Works First (OWF):
  • Apply online, or
  • Fill out the combined application form (other languages) and submit it to your County Department of Job and Family Services (CDJFS) office (for MBIWD, fill out the Addendum form as well).
• To apply for a PASS, contact your local PASS Cadre.
